For Children age 11 to 17 years old.  Take about 45 minutes. Parents can save their work and finish later. Includes a 16 to 30+  page report on your child including evaluation, and treatment and referral guidance. The report is generated entirely by a sophisticated , reliable, valid and useful computer program that has been in development since 1989.

The clinical screening report provides a detailed analysis based on your child's profile. Reponses to over 500 questions are processed, generating a report that includes the following information.

This Clinical Screening Report provides a reliable screening of teens by organizing and analyzing the reported history, observations and opinions of a child's behavior. This report provides a reliable screening for common psychological and behavioral problems. This report can be given to your counselor, social worker, psychologist or physician.

The results of your questionnaire are based entirely on information and data supplied by the person completing the questionnaires. The usefulness of the report is therefore dependent on the accuracy of the information entered to the questionnaire. Please take your time and answer every question carefully and to the best of your knowledge. Results are based on your answers.

The Clinical Screening Report is a tool for parents, appropriate family members and care-givers who want to screen a child's behavior and compare the screening results to other children. The Clinical Screening Report is designed to identify problems and help guide you in your next steps.

This report is for screening purposes only. This report is designed to help parents and caregivers identify potential problems in general terms. This report is not a professional evaluation or assessment.  It does not diagnose children or rule out medical problems. This report should not be used to rule out the risk of dangerous, self-harming or suicidal behavior. Further evaluation is essential especially when problems are identified. The Clinical Screening Report should be reviewed with you by a qualified health care professional.

Exclusion

InCrisis screening and report services provided by InCrisis are for children age 11 to 17 year. InCrisis services should not be used if the child suffers from any of the following:

  • Severe physical handicaps, including but not limited to the following: blindness, paralysis, loss of limb, deaf...

  • Mental neurological problems including but not limited to the following brain damage...

  • Significant physical defects, including but not limited to the following: missing limb, cerebral palsy, muscular dystrophy...

  • Severe medical problems including but not limited to the following: diabetes, cancer, multiple sclerosis, chronic pain, seizures, aides, internal organ damage or defects...

  • A medical condition requiring frequent or continuous medical monitoring

InCrisis services should not be used if

  • The user has a severe reading disorder and may have difficulty understanding written questions

  • English is the user's second language and he or she has any difficulty reading or understanding written English

  • The user has not interacted with and observed this child for at least 4 weeks

  • The user does not know this child or this child’s history

  • The child require frequent or continuous medical monitoring
