The Risk Analysis Report provides specific information and guidance regarding behavior and factors associated with a risk of violent, self-harming and suicidal behavior.  Includes an 8 plus page report on your child with a risk analysis.

Parents should go over the results with a qualified health care professional   or crisis intervention professional.

Requirements

• The Clinical Screening Report must be completed first.
